The mechanism explains the specificity. Arousal fills the pelvic vessels with extra blood; healthy veins drain it promptly afterwards. Veins with failed valves can't — so the congestion that follows fades slowly, as a heavy, dragging ache that can last hours or into the next day. Pain during sex has many possible causes; the lingering afterwards points at drainage.
